Register with us as a New Patient

Our practice accepts patients who live in the following postcodes: G1, G2, G12, G20, G21, G22 and G23.

If you do not live in any of these postcodes please do not complete a registration form as it will not be accepted.

When you attend the Practice to register you will be asked to complete a GP registration form, and new patient questionnaire form. These must be completed before you are seen.  If you are unwell you will be given an appointment to see the doctor, otherwise you will be given an appointment to see the Practice Nurse for a new patient medical.

Once you have been accepted as a patient, your medical records will be requested from your last practice if you were registered with an NHS practice previously.

This is to confirm you are registered with the practice, to allow the practice team to contact you and also to update your medical records held by the practice and our partners in the NHS. The internet is not a secure place, however, we have gone to great steps in making sure the information you submit to us is as secure as possible. We use SSL (Secure Socket Layer) certificates to encrypt the communication between your computer and our web server.
